I'm not quite sure what you're asking. Do you want a CGI that then 
generates a search form? I don't think this is necessary.

You can tailor the search form very easily--in addition to all the 
fields in the example form, you may also use the allow_in_form 
attribute (carefully) to allow additional attributes to be set in the 
form.

One note, you cannot currently restrict searches based on date.
