Not so unlikely, but a problem that is introduced quite often by
automatic cp generation. And these are cps that mess up the whole
project, so it is good to have a way to detect them. (The cp list is 
good therefore of course as well: those with the greates errors.)
I have always wondered why there is no algorithm in hugin that sorts 
them out automatically. (Cp generators can't yet because the project 
isn't optimized yet?) I think this should be an automatic step like 
running celeste before optimizing.
