You "should" use a tripod, but hugin can help when you can't.

As for the number of images, the bare minimum is that you need more bits
than the output.

In short, this means that you will output a LDR in the end, which is
typically 8 bit. So you need more than 8 as an input. Some cameras now take
raw in 12-14 bits per channel. This way, a single image "can" be used to
generate an HDR and tonemap it. By far not the best way, but 100% doable.
It's actually one of the best way to HDR a moving subject.

Typically people will use 3 images 0 and +\- 2 stops like you said (and oh,
only vary the time, not the Av) as you mentioned, but sometime, this is not
an option.
