What version of Hugin? On what machine? We need a bit more detail to check what went wrong.
Could you repeat the process and if it fails again, tell us *exactly* how you got there? Because this is a very unlikely result if you *only* pressed the "Stitch" button on the stitching tab after checking the alignment in the fast preview.
