To identify what is going on, you need to search syslog and see where your
volume goes offline.  
To find out the UCB of your volume have the command D VOL=xxxxxx (this is
the volser that went offline).  The display will contain the UCB address.

Then search syslog for a message that contains the UCB in that display

Or check to see if when your volume goes offline, if there are any IPLs,
backup jobs running, reorgs, or other dasd management jobs running.

Check with your system programmer and see if they have any SYS1.PARMLIB
members that might do vary offlines of your volumes.  Sometimes at IPL time
a member in SYS1.PARMLIB will have a V xxxx,OFFLINE and it might be brought
back on later by a job or operator.
