I received a response from the IBM'er working on my PMR on this issue:
Hi Glenn, There are nine different actions that can cause a notification to be generated for a tracked APAR. These events can occur in the same day. It is likely and in some cases very probable this can cause multiple notifications. Generally multiple PTFs are created for an APAR (for the different releases, platforms, etc.).
