I am not sure this qualifies as a "security breach" a bank here in
Chicago "lost" some tapes(3480 I believe) that had name & Address's of
depositors a few years ago.

I'd call it a security physical breach, caused by someone's sheer
stupidity. I remember it well as it made the local news.
