In 40+ years I worked for 8 companies, two of which were ISVs, two service bureaus, and the rest were contract software providers (mostly, but not all for government agencies). Many had strict procedures for how code was inspected and tested; one went so far as to reduce any abending production program back to test status, even when the abend was due to a hardware failure.

In all that time, I saw programs that evinced run-away space consumption, but they were all caught during testing.
