Yes I forgot to mention that selinux is off as well. I tried Marco's suggestion of turning off the authentication in cgi.cfg and was still prompted for a login - should that happen? After I turned it off I completely restarted apache, icinga & ido2db as well. I was using icingaadmin as the user, but tried changing * to allow all users anyway and still got the forbidden page. I guess icinga is probably functioning correctly I probably have a config issue in apache somewhere... Thanks for the help!

How about pasting a little apache config and the permissions of the used directories?
