from my personal experience when doing support, the best supported
platform is debian, next to centos. partly
ubuntu/fedora/suse/gentoo/*bsd/solaris/aix/...

debian mainly for the reason having a wonderful debian packager on the
team (formorer) and therefore getting the best out of 2 worlds - devs
and packagers. but still, there are other platforms as well which fit.
but still, we are seeking for official ubuntu and fedora package
maintainers.

you might wanna check pnp4nagios as well, taking perfdata via rrdtool
into rrds and integrate that into your preferred gui with shiny graphs
then.

how did you do that, and which versions are installed?

mh if it's ubuntu i'd stick with an lts, or drop it and install debian 6
instead. but that's your choice then.

might be misconfigured apache. some errors in the webserver logs?

action_url or notes_url from the hosts/services can hold some urls, as
well as you can integrate your own custom cronks with passing auth
information to nagvis itsself. check icinga-portal.org - iirc there were
some discussions on that integration already, and there might be more
users to help you out fiddling with that. but first you should try
yourself, as usual ;)

for the icinga parts - docs.icinga.org or wiki.icinga.org - if you need
the general setup and how to troubleshoot guides. as well as the wiki
holds some extended information. for nagvis, i'd stick with their docs
as well as the hope that someone shares his/her knowledge on that. i
don't use nagvis, so i'd keep my mouth shut on that topic when it comes
to more questions.
