OpenID examples using Tomcat 6.0.20 + WS02 does not work

It looks like documentation at
http://wso2.org/project/solutions/identity/1.5/docs/oprp_developer_guide2.html
is for an older version of WS identity server (1.5).
The attached JSPs and web.xml files are modified based on the samples seen on
Googles and WS02 site but it does not work (does not redirect to
OpenCallback.jsp even with setReturnURL set correctly). The JSPs compiled and
there is no issue with the return URL specified. There should be some error but
there is none on the console even with try-catch block put in place.
