I follow three things and its working well for me atleast on a couple of servers I adminsiter. [1] Remove anonymous edits [2] Allow registrations of new members with a capthcha. Mathematical captcha works well for me. [3] Take RSS feeds of Recent Changes page and keep a monitor on that. Ban the user posting spam, re-dit the wiki. Works well!
