I am able to fix that error. 
 
I downloaded HBA and RDAC drivers and installed after rebooting the machine. 
the problem got fixed.

will give you hint on disk performance.

Configuring SAN Box on RHEL - 5.0 giving the Buffer I/O error on device sdc

We have configured the SAN box(IBM DS3400. Arrays : 1 and logical Drives : 
1,LUN :9 fiber,4 hba's Emulex LightPulse Fibre Channel SCSI driver 8.1.10.3) to 
RHEL 5.0 system.

System able to detect the partition.

Whenever system starting or typing command fdisk -l below error are thrown in 
the /var/log/messages
