Only if your running the ezsignup templates etc. from the same "box" as your
imail server... then you need IIS 4 or 5 to serve those pages, otherwise no!
Your patching IIS not imail!

I know this seems like a silly question...but if you are using IMail on
a separate server from the one serving images via IIS, do you need to
worry about the patch on the IMail server?...

You might have many new 'random ip's' from the virus and your
application/service does not now what to bind too. Look at the nic's ip
settings.
