First, check the IMail log file to see how many "RCPT TO:" lines there are 
in there for this E-mail.  Does it have both "RCPT TO: user b" and "RCPT 
TO: user c"?  If so, it's a problem with the mail client on the sender's end.

If that isn't it, and there was only a "RCPT TO: user b", you should check 
that User B didn't accidentally forward the E-mail, and that they don't 
have forwarding set up in IMail.

If the fault is on your end, the logs will show the E-mail going from your 
server to User C, and you can work back from that point in the logs to get 
an idea of why it happened.
