Do you have a dns entry pointing to just domain.com at your ISP? We had
problems such as this until we added this entry. I know, I know that you
should not have to do this but spam protection was causing this even though
we had mail.ourdomain.com once we added a record (c-name not mx) for just
ourdomain.com it started working. We were having problems with AOL, Digex,
and bellsouth.net to name a few.
If you can get your hands on one of the returned emails look at the headers.
If it says host must resolve then this may be the case. And of course having
a ptr record (reverse lookup) so the host name resolves from the ip is good.

Has anyone had a problem using Virtual Hosts and AOL? I have a customer who
we have configured with a IMAIL virtural host. Whenever customers try to
send email to him, they get a message of host unknown. This doesn't happen
to everyone sending email, primarily AOL and few other sites. Any
suggestions?
