Does the client logout (close OE) on occasion? If the connection is not
terminated or not terminated correctly, IMail will not delete messages
or make any changes to the mailbox (so it does not 'lose' mail).

Sounds like the client index to the mailbox is getting corrupted. If you
remove that from the client side (I'm assuming this can be done), and
you login anew, is it OK? If you delete the client side account and
recreate it, does that help?
