[IMail Forum] Error when trying to change password

I saw this question asked before, but I never saw a cause

When I (or anyone) go to change passwords it returns to a Change Password
Status page that reports:
Error !! Domain does not exists

They can be changed without error from the Administration at which time it
reports:
Successful change of password for user "dan" in domain mail.lakehead.net.
We are running ver 5

What causes this and is there a cure?
