Jeffery,
   I've seen the exact same thing and identified the problem in
ODBCUSER.DLL.  I've reported it to Ipswitch Tech support and fixed it in
my own version of ODBCUSER.DLL.  

   This happens on a busy server where SMTP and POP3 start before SQL
server is ready.  If user hit the service and an auth fails, ODBCUSER.DLL
keeps the zombie connection around and never attempts to reconnect.
