Hi Daniel,
I just spoke with somebody from your support department earlier
today, and they said even with the IMail DB the files/folders do NOT get
created right away. That's the exact reason we called, a new domain that
was created had "Invalid Folder" listed 4 times, he had us send an email to
the account and all the folders were created and everything started working
correctly. We're using the IMailDB, not SQL.

Mailboxes (folders and files) will be created when each user is sent mail.
Only with IMail DB do the files and folders get created immediately.
