Imail does it fine, with a couple of reservations. 

1: get the 3rd party odbcuser.dll. 
2: create a separate database in SQL Server. Don't be tempted to use the
default db.
3: set the ODBC DSN to NT authentication (not SQL login) and make it default
to the database you created. Use a system DSN rather than a file one. (its
faster)
4: make sure that the NT account has the correct permissions on the database
before you start Imail.
5: Imail is flakey when creating a domain with an external DB. You first
create the domain with the external db setting then save it. Go back into
the settings and then configure the external db settings. Save it again.
Then and only then will it create the table in SQL and the associated "root"
user. 

6: there is a "feature" in Imail that doesn't create any indexing on the
table it creates. Manually create one for the userid field. This will speed
things up and also make sure you can't have duplicate entries.

7: Once its working you can create / update / delete users from SQL without
having to use Imail's adduser utility. We use an ASP script to replicate
users across 200 virtual domains this way, all sharing a common mailbox
folder.
