What we did is to insert the user information into the database and when this is
successful we run a batch file to create the sub-directory under your mail server for
that mailbox.
As long as you have the right permission this should not be a problem.
Notes:
I know that under SQL 6.5 you can create a insert trigger on the external imail
database that when the new user is created it can create a the sub-directory for you.
