I have it working with our internal billing database, but it took some
customization.   Although Imail doesn't explicitly state that they "work
with all known billing databases ever developed right out of the box without
any customization", it only makes sense that some work is required to
integrate.   There are some out of the box solutions provided by vendors
such as Emerald and Rodopi and maybe others.

   I ended up leaving IMail to its own separate database, and have a
procedure to keep them in sync via the command line ADDUSER when any change
is made to the billing database.   This allows us to designate some domains
as customer-managed where they manage all their own accounts via webmail.

   I also reorganized the Imail database so all domains appear in a single
table.   This is much easier to manage and allowed us to use a customized
version of OpenLDAP to create password protected access to LDAP.
Organizations with multiple domains can share their E-mail directories among
each other since they show up in a single table.
