Do you have "Ignore source address..." checked? Could be someone hand-jamming a URL to 
read the account.

Came up every minute for 2 days.  The user was not logged on to web
messaging, and no messages were sent.  There were no entries about
authentication or any activity. Happened over the weekend when there was no
one in the building.
