The mail client could be the problem, I saw a couple of times a strange behavior when the computer run out of disk space, Outlook express started to send the same mail several times, (10 or even more times)
In other occasion other machine did the same, but this time it had enough space, we had to reboot it to correct the problem. This was few years ago, I have not seen this again.
