I have found the answer to this is, PEBKAC (Problem Exists Between Keyboard And Chair.) I had a user that was doing several different things at the same time, not paying attention and adding people to the BCC: field. I did a full investigation after an embarrassing personal email went to about 20 people it shouldn't have and they insisted that it was the email server screwing up. The logs showed that they had indeed sent the email to all the people, and the original message in the sent folder had the BCC: field with the erroneous names. They now pay attention to what they are doing and the errors have stopped. The SMTP server has no way of randomly making up names to send to; those names come from the client.
Your problem may be different, but you can't ignore the human factor.
