Yes the url is calling a database that has case sensitivity turned on. I've used this in some of our web based surveys.

If you cut and paste the URL and type the last two characters- in lower case it won't work. When I first completed the form it worked and then I tried it again to see why it is not working then realized it is case sensitive. The last 2 characters need to be exact. Hmmm a URL that is case sensitive.. Interesting. Just in case that is the problem..
