Proper mechanism to configure CRAM-MD5 Password Authentication on UNIX

I've read the doc on md5.txt which states the file to be created is /etc/cram-md5.pwd.

How is this mechanism created? Is it done via saslpasswd?

Then copy the db over to /etc/cram-md5.pwd?

or is there another method of doing this?

I searched the docs but didn't find any examples.

I've configured IMAP-SSL for full compliance with IESG security requirements.
