The taskpane in infopath uses whichever IE is installed on the system.  You
can clear the IE cache (or used to be able to) from infopath by going to the
Tools menu and choosing Internet Options.

There are a few other versions of formentry out for openmrs 1.6+, see
https://wiki.openmrs.org/display/docs/FormEntry+Module

Did you just upgrade to 4.5.4?  Or did it just stop working on its own?  Be
sure to provide us all the details and backstory.
