Hi Marcos,

While in the form designer,

1) Set the data type for "State/Province" to "Single Select" and then use
the "Add Child" command to add the various "States/Provinces".

2) Set the data type for "District" to "Single Select Dynamic" and then use
the "Dynamic Lists" tab to add the various districts for each selected
"State/Province"

3) Set the data type for "City" to "Single Select Dynamic" and then again
use the "Dynamic Lists" tab to add the various cities for each selected
"District"
