I don't see any error on that screenshot you sent. For program, youjust want to enter a _single_ program name and a _single_ workflow name.

    wanted it which could be made optional if someone were willing to
    invest some time to do so.

    could just create a dummy program and dummy program workflow (I
    think you'll need to make sure you pick concepts of the
    appropriate type to associate with these), and then configure

        Widget Style from "Simple" to "Tree".  So this appears to be

            Location for the Order.  I just tried this out on an
            OpenMRS standalone running 1.8.2 and it added the order
            fine.  Did you choose a location from the Health Center
            dropdown?  You cannot leave this blank.
