I hope you are able to correct it.
My experience is it happens when cursor is inappropriately placed while
inserting the image resulting in link of the image getting deleted leaving
alone the thumbnail (this result in image not opening to larger size).
If you feel better, you may also shift to tabular format as designed by
Surajit ji.
