http://www.jems.com/jems/exclus05/e0531a.html
Experiences can often have negative outcomes by creating bad habits, instilling misinformation, establishing dependence on others, etc. However, these effects can been avoided by using a simple, three-question formula—What?, So what? and What next? LaMacchio, can help ensure emergency response teams are learning the knowledge and skills necessary for success and not learning other lessons that can lead to failure. For details, click here.
