Try putting PAM in lower case in this file. That may be the problem.
The built-in PAM support in SASL does not work well because Cyrus does
not run as root. You could try `pwcheck' with a PAM modification, or
`saslauthd' with the latest SASL release, which has PAM support.
Otherwise, turn on all the PAM debugging that you can find, and watch
the logs. Most PAM modules accept a `debug' option.
