Thanks -- but I see it has a value of 2, making me suspect that I am totally wrong about the cause.

Anyone have any good ideas?

I suspect that postifx is feeding cyrus faster than cyrus can take it. If that seems likely, is there a good way to throttle the flow?

try setting XXX_destination_concurrency_limit (where XXX could be local/virtual/etc, what you use to deliver to lmtpd) in main.cf.
