Why not simply add another imapd on another port in cyrus.conf which gets its own imapd.conf file with the -C option ?
Exactly. I always have services running on an alternate port. This is how I test code changes before I commit them to CVS or deploy them in production.

Nope. No different from the same user hitting multiple imapd/mailboxes at the same time (which probably happens a lot).
