I think you're problem is at least in part that you are including Apple's db.h, not the one from /usr/local/BerkeleyDB.4.1. Building cyrus-imap isn't exactly easy. I'd recommend trying the version from Fink first and see how they do it. After you've done that you can try to roll your own ...
