2.1.7 or 2.1.17? I just went through the trouble of configuring pam_mysql (0.4.7) for use with saslauthd (2.1.17) and it works fine with Cyrus 2.2.3. If you share your config, perhaps we can help.
Just out of curiosity, is there some reason why you need to use PAM, rather than using the SQL auxprop plugin?
