What do you see in your logs? You should see something similar to this, for
the Forward rule:

Feb  9 08:34:48 neo cyrus/lmtpunix[18087]: duplicate_check:
<1234567...@unionemail-cert.example.com>-pattyjo...@example.org
.pattyjo...@example.net.sieve. 0
Feb  9 08:34:48 neo cyrus/lmtpunix[18087]: duplicate_mark:
<1234567...@unionemail-cert.example.com>-pattyjo...@example.org.pattyjones@example.net.sieve.
 1265726088 0
Feb  9 08:34:48 neo cyrus/lmtpunix[18087]: sieve redirected:
<1234567...@unionemail-cert.example.com>-pattyjo...@example.org> to:
pattyjo...@example.org

And the rule which produced it:
require
["fileinto","envelope","reject","vacation","imapflags","relational","comparator-i;ascii-numeric","regex","notify"];
if
true
{
keep;
redirect "pattyjo...@example.org";
}

Do you see any errors in the logs?
