Generally, district productivity software, such as Office, Publisher, Acrobat,
etc. are paid out of my technology funds. Teacher requests for specific grade
level or department level software comes from their budget. If it is expensive
or will cover several dept or grades, I make an exception.

Regardless, teachers must complete an official requisition, submit it to the
Principal and then to me. I will then have a conversation with the teachers
about the purpose of the software and how it fixes into instruction. I had one
teacher who wanted to purchase 25 copies of a program at $100 each to do a one
day supplimental activity for her class. She thought that I should buy it.
She thought differently when she found out it was coming out of her budget. I
suggested that she purchase one copy, use a data projector and do it as a class
activity.

How do you guys handle requests from individual teachers for software
purchases? Do you have a process for addressing those? A form? Do you
require teachers to get approvals before purchasing?
