You need to un-comment line 90 in the file /webapp/jsp/jahia/htmleditors/fckeditor/fckconfig.js

89       ['Image','Flash','Table','Rule','Smiley','SpecialChar','PageBreak','UniversalKey'],
90   //  ['Form','Checkbox','Radio','TextField','Textarea','Select','Button','ImageButton','HiddenField'],
