  rdr pppoe0 202.76.166.28/32 port 412 -> 192.168.0.1 port 412 udp

Collapse that to

rdr pppoe0 202.76.166.28/32 port 412 -> 192.168.0.1 port 412 tcp/udp

Or even

rdr pppoe0 0/0 port 412 -> 192.168.0.1 port 412 tcp/udp

if the IP address changes.

  the tcp side is making a connection
  but the udp is being blocked.

What does ipmon -o N or ipmon -a say? Have you tried ipftest?
