You're not limited to 57x57px. I took my 128x128px icon prepared for
my Apple Web App listing (my "old" Tip Calculator) and put it into the
app's directory (also using the <link> tag as noted earlier). The icon
may be scaled (and obviously not as efficient as a smaller one), but
looks good enough for a first stab. Tomorrow I'll look into making
sure it works for the data: protocol "local" version.

BTW, the iPhone appears to add the mid-button swoop shading (I forget
what they call the effect in PS) to these icons.
