I feel uncomfortable with a "thread per operator" model, where there is one 
call to "run" for the duration of the whole query. I'd rather use co-operative 
multi-tasking: the "run" method is called multiple times, and each time it 
reads a consumes a batch of rows and produces a batch of rows.

This solves your problem, because an invocation of the "run" method would see 
no input rows, and perhaps a sentinel row indicating that the operator should 
close.

Other benefits are the system is more thrifty with threads, allows a scheduler 
to potentially optimize so that adjacent operators are running on the same 
core, and potentially reduces the amount of contention for shared data 
structures.

The downside is that these operators are a bit more difficult to write 
(especially when there is state that needs to be persisted across invocations).
