Ian, give this a try again and see if my fix worked. I think the problem is 
that your custom UI wasn't removed from the DOM after the dialog closed, 
causing the password confirm to incorrectly point to one of the old forms, in 
some cases -- I couldn't reproduce consistently.

When using the standard widgets (i.e., the built in createForm: {} options) DOM 
cleanup should be handled automatically, but with custom widgets you need to 
explicitly destroy any UI, or at least verify to make sure it was removed from 
the DOM via Firebug, i.e, after the dialog closes make sure there are no 
instances of $('.account-wizard') left.
