I think that confirmation dialogs (rendered via dialog.confirm) have a
'confirm' (green tick (/)) icon, not a 'warning' (exclamation mark) icon.
Please confirm that is the green tick icon that we want to use for
dialog.createForm rendered in confirmation style mode.
