I'm taking a guess here, but I think it's this Plugin - 
https://github.com/apache/cordova-plugins/tree/master/file-system-roots

The current dev branch of camera fixes this issue, but here's the gist of it. 
The camera saves to the root filesystem and by default Cordova doesn't have 
access to it. So you need to add the file-system-root plugin to allow the right 
permissions.
