  I think that you should check the region file of table the hole and the 
region in the hole, you can create region for this hole.
  otherwise you should not create region. 
  There is scenarios you shou think: the region of table is good or this region 
is junk file.

2.FIRST_REGION_STARTKEY_NOT_EMPTY and HOLE_IN_REGION_CHAIN
  I think when there is a type of error you can create empty region for this 
hole.
  if there is another error, another error you handle the first.
