htraced: fix some metrics issues.
* Add IngestedSpans metric to keep track of how many spans in total have been
ingested from all remote addresses.
* Track remotes by address rather than by address:port. Ephemeral ports make
the latter impractical.
* Fix a bug where the HRPC endpoint was not passing through some client stats
