Review Comment:
   Options allow to override the behavior with different INSERT INTO statements 
on same table created using CREATE TABLE with it's properties.
   
   I don't see need for that for database or table name fields. I think keeping 
them explicitly scoped to only being passed in row or pass in create table 
properties should avoid confusion.
